Audit
Each congregation is required to submit an annual audit of accounts
to the diocese. The following documents provide helpful information
regarding audit requirements:
Diocesan Information Form
This form relates specific contact information about your parish, its employees and its leadership. It is due annually within 15 days of your parish's Annual Meeting.
Directions:
1) Please make every effort to complete the form using a computer. This helps to eliminate errors due to illegible handwriting—especially important with email addresses.
2) At the very minimum, Adobe Acrobat Reader must be installed on the computer you wish to use to complete the form. Adobe Acrobat Reader is a free program. Download Acrobat Reader
3) Unless the computer you use to complete the form has the full version of Adobe Acrobat (not a free program) installed, you will not be able to save the information you enter. (The computer will allow a save, but when you reopen the document, the information you entered will have disappeared.) Therefore, you need to plan sufficient time to complete both pages at the same time.
4) To enter text on the form, place your cursor in the appropriate text field and click. Once you have entered all information on the form, do not attempt to save or close the form before printing a hard copy.

You may complete the form using your computer, you have the option of returning your form to the diocese via email. To begin, click on "Select Text" in the toolbar to activate your cursor. Then enter the requested information. It is recommended that you print a copy of your completed from before you attempt to save it. Then use the “Save As” command and retitle the form to reflect your church’s name. (i.e. St. Swithen’s 05 Info) Cloee the file, then reopen it to insure your responses were saved properly.Then, attach the form to an email and send to:

If your church office does not have a computer, and no member of the congregation can complete this task for your church using their home computer, you may call the Communication Office at 716-881-0660 x22 to make other arrangements.
Parochial Reports
All congregations must file an annual parochial report with
both the National Church and the local diocese.
A congregations may now opt to complete
the annual parochial report online and transmit it directly to the
National Church in electronic form. If this option is selected, the report
must also be printed, signed and mailed to the diocese in order to meet
local canonical requirements for the seating of lay delegates to Diocesan
Convention.
Congregations not equipped to file electronically will completely satisfy
the requirements by filing a hard copy of the parochial report with the
diocese in the same manner as previous years. Please attach a note stating
that national filing has not been accomplished. (The diocese will then
provide your congregation’s information to National Church for you). Mail

Preparation responsibility: The report is the joint responsibility
of the Rector and Vestry of every parish and of the Vicar and Warden of
every mission church (Canon 13, Paragraph 3).
Signatures: The report requires certification by all of the following:
- Rector/Vicar/Person in Charge
- Clerk of the Vestry/Council and
- Treasurer/Financial Officer
Extensions:
If you will be unable to complete your report by March 1, please
phone Kathy Williams at (716-881-0660 x.12) to discuss the status and
obtain an extension to May 1.
Acknowledgement of Receipt:
Shortly after you send your completed parochial report to the diocese,
you will receive a postcard acknowledging its receipt. This will be sent
to the rector/vicar/person in charge. If you have not received a postcard

Theological Education Report
All parishes are asked to make an annual donation in support of theological education.
